There has been a report by the 37 Military Hospital that the ultrasonic scan performed on a woman prior to delivery was error-ridden.
According to the hospital, the error was discovered after further investigation into the missing twin.
The Ghanaian Times’ investigative story published on January 18, 2023 led to this disclosure five days later.
In response, the hospital apologized to the couple and promised to release the results of its investigation this Friday, saying that the incident should serve as a wake-up call for the facility.
A briefing of senior military officers was held on Monday at the Burma Camp in Accra with the Chief of Staff (COS) of the Ghana Armed Forces (GAF) – Major General Nicholas Peter Andoh and the family of Daniel Naawu, the husband of the woman who was expecting twins.
At the meeting, which lasted a little over an hour, Maj Gen Andoh invited Brig. Gen., a top military officer. Among the members of the team investigating the issue were Major Alfred Topper and Raymond Kwabena Ewusi, Director-General of Ghana Armed Forces Medical Services, Navy Commander Dr Paul Yegbe, an Urologist and surgeon at the 37 Military Hospital.
At the meeting, Dr. Ali Saine, who performed the Caesarean Section (CS) on that fateful day (September 8, 2022), also answered questions.
John Naawu, the legendary Ghana goalkeeper, led the four-member family in bombarding Dr Saine with questions about the supposed missing twin and an ultrasound taken after 31 weeks.
Despite the scan showing twins, the hospital was surprised to see only one newborn, according to Dr Saine.
According to him, Ms. Audrey Agyapong’s condition was caused by a faulty diagnosis at Cerica Diagnostic Services in Accra.
Dr Saine acknowledged that the second scan performed at the 37 Military Hospital also contained errors, insisting only one baby was born.
The hospital’s chief executive, Maj Gen Andoh, said there was no intention to do anything malicious since credibility and reputation are always at stake. “Anyone who damages the reputation of the military will not be protected.
Despite expecting twins, the couple was handed one baby after delivery from 37 Military Hospital, leaving them suspicious of foul play.
According to Dr Eric Otu-Danquah, the Technical Director and Radiologist at Cerica Diagnostic Centre, his organization did its due diligence before determining the result.
Neither institution denied the existence of human error. Even though 37 Military Hospital has stated that human error will guide its future healthcare delivery, the Cerica Diagnostic Centre still believes it gives diligent service.
Dr. Otu-Danquah says “It’s unfortunate that 37 Military Hospital came to that conclusion”. I have my contact number on the scan report in the possession of Ms Agyapong; so they could have called to cross check if they had any doubts.”
The first scan report which was signed by Dr. Otu Danquah, was conducted on April 22, 2022, at Cerica Diagnostic Services when the pregnancy was 20 weeks old. Ultra-sound checks done 31 weeks into the gestation period at two medical facilities, including the 37 Military Hospital, showed that one Ms. Audrey Agyapong was carrying two live fetuses (twins).
All scans showed she was carrying live twins. It is baffling that one could disappear at birth that baffles her.
