Can HIV Be Transmitted Through a Human Bite? What Medical Evidence Says

Concerns about HIV transmission can arise after a human bite, particularly when the incident involves a police officer, healthcare worker or emergency responder. However, medical evidence shows that contracting HIV through a bite is extremely rare.

According to current guidance from the Centers for Disease Control and Prevention, human bites without blood exposure are not considered an indication for HIV post-exposure prophylaxis. Reported cases of possible bite-related transmission have been exceptionally uncommon. CDC guidance

HIV Transmission Risk Is Extremely Low

Ordinary saliva is not considered an effective route for HIV transmission. Although tiny amounts of the virus may sometimes be present, saliva does not contain a sufficient concentration to transmit HIV under normal circumstances.

Saliva also contains enzymes and proteins that inhibit the virus. Consequently, being bitten without exposure to visible blood generally presents no meaningful HIV risk.

A bite that does not break the skin cannot transmit HIV. Even when the skin is broken, transmission remains extremely unlikely unless blood from the person biting enters the wound.

When Could a Theoretical Risk Exist?

A possible risk would require an unusual combination of circumstances. These could include:

  • The bite caused a deep wound, severe tissue damage or significant bleeding.
  • The person biting had visible blood in their mouth because of bleeding gums, sores or recent oral trauma.
  • The blood entered the injured person’s broken skin or bloodstream.
  • The person biting had HIV with a detectable viral load.

People receiving effective antiretroviral treatment may have an undetectable viral load, which dramatically reduces transmission concerns. Nevertheless, healthcare professionals should evaluate any serious blood-involved bite individually.

What to Do Immediately After a Human Bite

A wound that breaks the skin should be washed promptly and thoroughly with soap and running water. Harsh chemicals should not be poured into the injury, and deliberately squeezing or cutting the wound to make it bleed is discouraged.

The injured person should then seek urgent medical care, especially if the bite is deep, visibly contaminated with blood or located on the hand, face or near a joint.

Medical evaluation is important because human bites can transmit bacterial infections more readily than HIV. A clinician may also assess the need for antibiotics, tetanus vaccination and testing or preventive care for hepatitis B and other blood-borne infections.

When Is HIV PEP Considered?

Post-exposure prophylaxis, commonly known as PEP, is a course of HIV medication taken after a possible exposure to prevent infection.

PEP is not routinely recommended for bites involving only non-bloody saliva. However, it may be considered when the bite caused severe trauma and visibly bloody saliva from a person with known or suspected detectable HIV entered the wound.

When PEP is appropriate, it should be started as soon as possible—preferably within 24 hours and no later than 72 hours after the exposure. A qualified clinician must make the final decision after reviewing the circumstances. CDC PEP guidelines

Testing and Follow-Up

An occupational health physician or emergency clinician may recommend a baseline HIV test following a potentially significant exposure. Additional tests may then be scheduled according to the type of exposure and whether PEP was prescribed.

The source person’s HIV status, treatment history and viral-load information may assist the assessment when those details are lawfully and voluntarily available. Treatment should not be delayed while waiting for test results if a clinician determines that the exposure warrants PEP.

Human Bites Should Still Be Taken Seriously

HIV transmission through a human bite is extraordinarily rare and generally requires direct blood exposure combined with significant injury. A bite involving saliva alone is not regarded as an HIV transmission risk.

However, any bite that breaks the skin should receive prompt medical attention because of the possibility of bacterial infection, tissue damage, tetanus and exposure to other blood-borne diseases.
